CF&R Form Instructions

Please follow the procedures below to insure your remodeling request is processed and received by the Capital Facilities & Remodeling Committee. If you would like to submit a request for a new building, please call Betty Martin at 585-6751.

1.

Print a copy of these instructions for reference while completing the request form.

2.

All requests should be submitted by the cognizant Vice President once all approvals are complete. Please, do not click the "Submit & Print" button until all approvals have been completed.

3.

Open the CF&R form by clicking on the link below (Requires Acrobat Reader 7 or 8).

You can obtain the free Adobe® Reader® at http://www.adobe.com/products/acrobat/readstep2.html.

To highlight required fields, click the Highlight Required Fields button in the upper-right corner of the form.

5.

You must complete all required fields prior to submitting your request.

Note:  Please use the comment boxes provided rather than attaching additional pages with your request.

If additional data needs to be gathered, you may save a copy of the form and reopen it at a later time to add additional information.

You may also print a copy of the document by clicking the Printer icon or File>Print on the Acrobat Reader toolbars.

8.

After completely filling out your request, review it for completeness and accuracy.

Forward the electronic copy of your request to your department chair, dean, or director for approval by clicking the "Forward To:" button on the upper-left portion of the document. Add the appropriate e-mail address(es) to the "To:" field and send the document via e-mail. They may write additional comments if they wish. It is the responsibility of the Dean/Director to collect all requests in his/her area, prioritize, and then electronically forward the documents to the cognizant Vice President.

The cognizant Vice President must approve and designate the project as either "Remodel" or "Research" and prioritize it within its respective category. After review and final approval, click the "Submit & Print" button to electronically submit and print a copy of the form.

11.

Scan or photocopy only pertinent backup or supportive information that cannot be included on the form. If copied, please double-side your copies to reduce the amount of paper and highlight information to be noted. Supporting documents should be transmitted under separate cover.

PLEASE DO NOT SEND EXCESSIVE INFORMATION.

12.

The cognizant Vice President is to submit all completed and approved CF&R requests electronically and supporting documents by January 11, 2008 to:
